This form will allow you to upload text and image files for submission to the Journal. Return to the Welcome Page for directions on using this feature. Consult the Instructions for Authors for details on preparing your text files and images. Please be certain to include your name and contact information for acknowledgment. After submitting your files, a confirmation page should appear that begins: "Thank you for your submission to the Journal...." ; if the page you see after upload has completed is different, the submission has failed. If your problem is a server time-out, you likely have too large a set of files in a single upload session. Try using multiple upload sessions to send only a couple of files at a time. For multiple upload sessions, note in comments that additional files will be sent. Use multiple upload sessions when you have >5 files to transmit or the individual files are too large to be transmitted by your server in a single upload session. To enter a file name, browse through your computer and select the file. The complete file path should appear in the box (e.g., C:\My Documents\manuscripts\stent trial). Repeat the process for up to 5 files. NOTE: do not include special characters (e.g., *, &, #, /, (), [], etc.) in the file name, which should be less than 40 characters in length!
